New patients should plan to arrive 15–20 minutes early to complete intake forms.

Your first visit will include a thorough review of your medical history, a physical examination, and a discussion of your symptoms and concerns. Based on that, your doctor may order additional tests such as bloodwork or imaging. Plan for your visit to take a bit longer than a routine follow-up — we want to make sure we get the full picture.

Call our office at (386) 561-9967 as soon as possible to reschedule. Please note that cancellations made less than 24 hours in advance are subject to a $50 fee.

Yes. All patient information is protected in accordance with HIPAA regulations. Your records will never be shared without your written consent, except as required by law.
